CITY OF TRACY – REQUEST FOR PROPOSALS NOTICE OF REQUEST FOR PROPOSALS 1. NOTICE IS HEREBY GIVEN that the CITY OF TRACY (hereinafter "CITY") requests proposals for COMMUNITY SUPPORTS QUALITY OVERSIGHT AND OPERATIONAL COMPLIANCE CONSULTANT (hereinafter “Consultant”) and sealed proposals will be received at the following location: City of Tracy, Parks, Recreation and Community Services, 333 Civic Center Plaza, 1st Floor, Tracy, California 95376, ATTN: Virginia Carney, Homeless Services Manager up to the hour of 5:00 p.m., on the 26th day of March 2026. 2. The services to be performed by the successful Proposer are described in the Request for Proposals (RFP). Copies of the RFP are available on the CITY website at: https://www.cityoftracy.org/I-Want-To/View/Bids-and-RFPs. The general scope of services includes providing independent program monitoring, service authorization support, claims and billing quality assurance, data analytics, compliance oversight, and continuous quality improvement for the City of Tracy’s CalAIM Community Supports programs, including but not limited to: • Recuperative Care (Medical Respite) • Short-Term Post-Hospitalization Housing (STPHH) • Housing Transition Navigation Services The Consultant will ensure program operations align with the California Department of Health Care Services (DHCS) Community Supports Policy Guide and applicable Medi-Cal Managed Care Plan (MCP) requirements. Questions regarding this RFP shall be submitted via email to Virginia Carney, Homeless Services Manager, at Virginia.Carney@cityoftracy.org no later than 5 p.m. on Thursday, March 12, 2026. 3. Any changes to the RFP will be posted on the CITY website in the form of an addendum. PROPOSERS shall periodically check the CITY’S website at: https://www.cityoftracy.org/I-Want-To/View/Bids-and-RFPs for any Addenda issued for this request and shall be acknowledged in the proposal documents. 4. Proposals shall be submitted and clearly marked as follows: “SEALED PROPOSAL FOR COMMUNITY SUPPORTS QUALITY OVERSIGHT AND OPERATIONAL COMPLIANCE CONSULTANT – DO NOT OPEN WITH REGULAR MAIL.” 5. All responsive proposals shall be reviewed and evaluated by the City in order to determine which proposer best meets the City’s needs for this Project by demonstrating the competence and professional qualifications necessary for the satisfactory performance of the required services. The criteria by which the City shall evaluate proposals are set forth in the Request for Proposals. 6. The City reserves the right to reject any or all proposals and to suspend, amend, or modify the provisions of this RFP, and/or waive any irregularities in the proposal process, this RFP document, or the City’s selection process.
