The Goal Of This Program Is To Expand And Improve Primary Health Care Access And Services Reduce Unnecessary Utilization Of Hospital Emergency Services And To Encourage Collaborative Relationships Among Community Based Health Care Clinics Hospitals And Other Health Care Providers
