Description

Seeking to contract with one or more experienced community agencies to provide case management for individuals with disabilities and older adults who receive funding through one of the following home and community-based services programs and/or county funded case management community access for disability inclusion (cadi), community alternative care (cac), brain injury (bi), developmental disabilities (dd), non-waiver county funded rule 185, elderly waiver (ew) or the alternative care (ac) program. Q&A deadline On or before Friday, February 27, 2026 at 4:00 P.M. (CST). Contract term From April 1, 2026 through December 31, 2027. This solicitation has some or all Federal funding, additional requirements may apply.
