The program will focus on establishing a positive behavioral support program within the family home, training family members on the program, modeling the program for the family, and supporting the family, in setting up appropriate, on-going in-home supports. The Community Clinical Response can be provided for up to 180 days per individual. DDS is seeking proposals from private providers to develop, operate, supervise and manage the administrative duties associated with the operation of this program. The successful proposer will be required to provide supports within the family home, complete a functional behavioral assessment (FBA), develop a positive behavior program, train the family on implementing the plan, model program implementation and provide consultation to other in-home support providers, and develop a transition to discharge. They will also be required to provide nursing consultation, if needed, regarding medication and medical needs. Contracts shall be issued with at least one (1) selected vendor for fiscal intermediary services. DDS participants that utilize FI services may be provided with a choice of which vendor they would receive FI services from.DDS shall issue contracts with at least one selected vendor for fiscal intermediary services. DDS is looking to open ten (10) new CLAs. Nine of the CLA’s are for young males with a history of behavioral health needs. The tenth CLA is a home for medically fragile individuals who require tracheotomy care. It is proposed that each region will have three homes which will specialize in supports for young behavioral challenged individuals. The medical home that will specialize in care for medically fragile individuals will have 4 beds and will be required to be located in the Central Connecticut area. Each home will range from three (3) to five (5) beds. The qualified vendor, or an agency that can become a qualified vendor, must have experience working with individuals who have intellectual disabilities and must have experience providing the supervision, administrative oversight and operations of a 24-hour Licensed Community Living Arrangement. The qualified vendor, or an agency that can become a qualified vendor, may bid on as many or as few homes as they feel they can appropriately support. The deadline for submission of the proposal is Friday June 13 2025, by 4:00 p.m. RFP #202510 NR-WR-SR CLAs CLA Budget Summary Questions & Answers for RFP #202510 NR-WR-SR CLAs FY25 RFP # 28988 DDS South Region Emergency Transitional Home 01 The State of Connecticut, Department of Developmental Services (DDS) is seeking proposals from DDS qualified providers to develop, operate, supervise and manage the administrative duties of a transitional housing program for individuals over the age of 18 who have been diagnosed with intellectual disabilities, co-occurring mental health issues, and have experienced involvement with the criminal justice system. This facility could also be used as an interim step for individuals being discharged from a locked forensic unit. Based on capacity, admission could be expanded to serve individuals who are discharged from incarceration, involved with probation and parole, homeless or anticipate homelessness, have an unsuccessful residential placement, have probate involuntary placement, have loss of caregiver due to death, illness or other loss (eviction, foreclosure), unnecessary emergency admission, or unnecessary hospitalization. The program will be in state owned facility in Meriden. The successful proposer will be responsible for providing support for behavioral, mental health, and legal needs to individuals of the Department. They will maintain the health and safety of the residents, safeguard the individual funds of the residents, and follow the policies and procedures of DDS. Please be advised that the successful proposer will be required to assist program residents to secure permanent housing and/or residential supports within 180 days. Extensions to this time frame may be granted by the Regional Director. DDS will work with the provider to establish clear criteria for timely admissions to and discharges from the program.
