MRC ACQUIRED BRAIN INJURY/MOVING FORWARD PLAN TRANSITIONAL ASSISTANCE SERVICES

The Massachusetts Administrative Office (AO), on behalf of the Massachusetts Rehabilitation Commission (MRC), has released RFP #BD-18-1071-AO-CON01-22804 for Acquired Brain Injury/Moving Forward Plan Transitional Assistance Services. The purpose of this RFP is to solicit proposals from qualified organizations to provide transitional assistance services to individuals with acquired brain injuries participating in the Moving Forward Plan. The scope of services includes, but is not limited to: support and resources for community housing, group homes, shared and assisted living. Key requirements include demonstrated expertise in supporting individuals with acquired brain injuries, experience navigating housing options, and the ability to provide individualized transitional support services. Vendors must complete and submit all required documentation, including the Contracting Qualification Form and Contractor Authorization Signatory Listing, with their proposals.
