Description

Seeking a qualified agency to provide all services regarding disability application assistance and advocacy for participants of the program. This is to include assistance with and coordination of health care, including behavioral health care, for participants as appropriate, assistance for participants in gathering documentation needed for the disability application process, as well as documentation of all case work throughout the process with each participant from initial application, reconsideration proceedings, if necessary, to benefit receipt or final denial. This solicitation has some or all Local funding, additional requirements may apply.
