The NY HIV Planning Council has directed that a total of $21,096,845 ($17,750,695 Base and $3,346,150 Minority AIDS Initiative (MAI) of the annual HRSA RWHAP Part A NY Eligible Metropolitan Area (EMA) funding amount be allocated for Care Coordination Services for People with HIV in NYC annually, a total of $1,333,800 for Outpatient Ambulatory Services for People Aging with HIV in NYC annually, and a total of $1,349,825 for Emergency Financial Services for People with HIV in NYC annually for procurement through this RFP. PHS, on behalf of the NYC Health Department, is soliciting proposals from eligible applicants in response to the three new NY Planning Council directives via three service categories: • SC1: Care Coordination Services for People with HIV in NYC (Anticipated Number of Subawards: 17) • SC2: Outpatient Ambulatory Services for People Aging with HIV in NYC (Anticipated Number of Subawards: 3) o In order to apply for this service category applicants must also apply for Care Coordination Services for People with HIV in NYC (SC1) o Applicants will have an opportunity to enhance care coordination services by providing outpatient ambulatory services to people aging with HIV (defined as 50 years of age and older). • SC3: Emergency Financial Services for People with HIV in NYC (Anticipated Number of Subaward: 1)
