Request for Proposal: WholeYouNYC Social Care Network Home and Asthma Remediation Health-Related Social Need Services: Home and Asthma Remediation Coordinating Provider

Public Health Solutions (PHS) is issuing this Request for Proposals (RFP) to identify organizations to coordinate the delivery of Home and/or Asthma Remediation Health-Related Social Need (HRSN) services to eligible regional Medicaid members (“Members”) under the WholeYouNYC Social Care Network (SCN). Specifically, PHS seeks proposals from applicants interested in serving as: Home and Asthma Remediation Coordinating Provider (“The Coordinating Provider”) The Coordinating Provider(s) will contract directly with the WholeYouNYC SCN to either coordinate the delivery of home and asthma remediation services, including providing navigation support and managing relationships with remediation subcontractors or to provide technical reviews of Statements of Work and quality assurance inspections. Coordinating Providers may also provide direct remediation services under this contract. *Note - Home and/or Asthma Remediation Direct Service Providers: Applicants interested solely in providing remediation services (such as mold removal, ventilation improvement, or other environmental interventions) should apply to the separate Request for Qualifications (RFQ) titled Pre-Qualified List for WholeYouNYC Social Care Network (SCN) - Health-Related Social Needs (HRSN) Enhanced Services: Home and/or Asthma Remediation Direct Service Provider (Solicitation #2025.08.SCN.07.01)
