Request for Qualification: Pre-Qualified List for WholeYouNYC Social Care Network (SCN) - Health-Related Social Needs (HRSN) Enhanced Services: Home and/or Asthma Remediation Direct Service Provider

Public Health Solutions is seeking to pre-qualify independent contractors and organizations who meet the eligibility requirements to deliver home remediation and/or asthma remediation services for our WholeYouNYC Social Care Network (SCN). These services are critical to improving housing-related health outcomes for Medicaid members. Independent contractors and organizations with demonstrable experience in providing these services in New York City will be considered for inclusion on an SCN Pre-Qualified List (PQL). Those selected shall become pre-qualified partners that will be eligible for potential future engagements/assignments to provide these services which may include a requirement to enter into an agreement directly with a contracted HRSN Service Provider. This PQL will ensure a robust, responsive vendor network that can support the SCN’s goal of delivering high-impact, housing-based interventions that improve the health and well-being of New York’s Medicaid population. The Pre-Qualified List (PQL) will support the SCN’s efforts to: • Ensure Service Quality and Compliance: Only providers with the appropriate credentials, training, and experience will be eligible to deliver remediation services. • Promote Transparency and Accountability: A vetted pool of service providers will help uphold consistent standards for safety, member health outcomes, and program integrity. • Increase Efficiency in Service Delivery: Pre-qualification will streamline the referral and contracting process, enabling faster response times to members with identified health-related social needs (HRSNs)
