Professional Chef - Public Speaker

Offerors must submit a bid that meets the requirements identified in this ITB. See attached documents and below. Cost per presentation – Video conferencing or in person up to 1.5 hour(s) per speaking engagement to allow for Q & A.OFFEROR REQUIREMENTS: The Offeror bid document must meet each of the following requirements:Must have/show 10+ years as a professional Chef Must have/show experience as a Lead Chef at a restaurant (not fast food) Must have a valid state driver’s license or state ID w/picture Present in person or via Video Conference Must provide a brief description of what will be covered in the proposed Speech All bids must contain the following: Business Name (if applicable) or individuals name, contact person, address, phone number, email, location(s) willing to provide services to (listed above), cost, years of experience, at least 1 reference and provide a completed/signed W-9.DELIVERY LOCATION: Speaker will provide 1 engagement per site in person or by video conferencing for the following locations (subject to change)London Correctional Institution, 1580 State Route 56, London OH 43140 Madison Correctional Institution, 1851 OH-56, London, OH 43140 Pickaway Correctional Institution, 11781 State Route 762, Orient, OH 43146 or any additional sites agreed upon between all parties
