MRC ACQUIRED BRAIN INJURY/MOVING FORWARD PLAN TRANSITIONAL ASSISTANCE SERVICES

The Commonwealth of Massachusetts, through its Administrative Office (AO), has issued RFP #BD-18-1071-AO-CON01-22804 for Acquired Brain Injury/Moving Forward Plan Transitional Assistance Services. The primary purpose of this RFP is to solicit proposals from qualified organizations to provide transitional assistance services to individuals with acquired brain injuries, specifically in support of the Moving Forward Plan. The scope of services includes, but is not limited to, developing and implementing individualized transitional plans, providing housing support, and offering technical assistance. Key requirements include the ability to deliver services to individuals residing in group homes, shared living arrangements, and assisted living facilities. Bidders must demonstrate experience in working with individuals with acquired brain injuries, a comprehensive understanding of transitional service planning, and the capacity to track and document housing support activities. Detailed plans for technical assistance delivery in group homes, shared living, and assisted living settings are required, as well as the use of a standardized Housing Support Log.
