EOHHS plans to submit a grant application to the Centers for Medicare & Medicaid Services in response to the federal Rural Health Transformation Program, Notice of Funding Opportunity. EOHHS is announcing through this Notice of Intent, 1) the rural health facilities it intends to partner with to meet the strategic goals of the funding and 2) the definition it used to determine the rural health facilities, target populations and geographic areas
