Provider Agreement (PA) - Adult Individualized Services Program

The Department of Health (DOH), Division of Behavioral Health (DBH) is entering into Agreements under 7 AAC 81 with qualified Providers for the State of Alaska’s Adult Individualized Services Program (AISP) to provide community-based services to Adults experiencing Serious Mental Illness (SMI) and/or Substance Use Disorder (SUD).
