OPWDD is issuing the attached Regional Disability Health Clinic Program (RDHCP) Request for Applications (RFA). The goal of this grant opportunity is to improve health care for people with developmental disabilities by increasing access to care and better coordinating and integrating services. The RDHCP will provide grants in support of capital projects to expand access to quality health care for people with developmental disabilities by reducing accessibility barriers and increasing access to healthcare services, including medical, dental and other physical health services such as those provided at Article 16 Clinics (e.g., dental, medical, nursing, physical therapy, occupational therapy, speech and language pathology) and Article 28 Clinics (e.g. medical, dental, nursing). This opportunity is available in the Statewide Financial System (SFS) and can be found by searching forSFS Event ID: OPD-RDHC-1. Important Dates -Updated on September 26, 2025 Minimum Qualifications: Per Section 1.3.1 of the RFA, the Applicant must be a: 1.3.1.1.1. Clinic Treatment Facility certified or licensed under Article 16 of the Mental Hygiene Law (MHL), 1.3.1.1.2. Diagnostic and Treatment Center licensed under Article 28 of the Public Health Law (PHL), excluding end-stage renal disease facilities and ambulatory surgery centers, 1.3.1.1.3. Federally Qualified Health Center (FQHC) licensed under Article 28 of the Public Health Law (PHL), RFA OPD-2025-49 6 of 27 Regional Disability Health Clinics Program 1.3.1.1.4. Rural Health Center (RHC) licensed under Article 28 of the Public Health Law (PHL), or 1.3.1.1.5. Hospital extension clinic (off-campus site) licensed under Article 28 of the Public Health Law (PHL). 1.3.1.2. The Applicant must be a not-for-profit organization. 1.3.1.3. The Applicant must provide a current, valid copy of a New York State Operating Certificate for the relevant Article 16 or 28 location. 1.3.1.4. The Applicant must not be on the OPWDD Early Alert list (https://opwdd.ny.gov/providers/early-alert). 1.3.1.5. The Applicant must prequalify through the Statewide Financial System (SFS) by the application due date and time. NOTE:Applicationsmustbe submitted in the Statewide Financial System (SFS) by the due date and time.Late applications will not be accepted under any circumstances. Applicants are strongly encouraged to submit applications 24 to 48 hours prior to the due dateto avoid the risk of ineligibility resulting from unanticipated delays or other computer or technical problems.
